Diabetes is a chronic disease that affects people of all ages. It has serious impact on the health and lifestyle of people suffering from the disease. Diabetes also affects other parts of the body and diabetes-related health complications can result in serious consequences, affecting the individual’s quality of life. The multi-system effects of diabetes include heart disease, eye disease, dental problems, chronic kidney disease, nerve damage, and foot related health problems.
Patients will be followed by a multidisciplinary team of healthcare providers and staff for coordination of care for a duration of program. This comprehensive program will include diabetes education and lifestyle modifications.
In the end, patients in this program have a goal to reduce A1C by 15%, reduce or eliminate medications for diabetes as well as other chronic disease, have a consistent blood pressure of less than 140/90 and experience weight loss.
Patients will achieve their individual health goals and improve behavior and healthy living.
